Online Enrollment
CareMore Health Plan is a Medicare Advantage Organization with a Medicare Contract. This Medicare contract is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed.
Prospective members may enroll in CareMore Health Plan during the following enrollment periods. For more information please contact CareMore Health Plan at 1-877-211-6614.
Enrollment Periods:
• Annual Election Period (AEP): November 15 through December 31
• Open Enrollment Period (OEP): January 1 through March 31
• Special Enrollment Period (SEP) for limited special exceptions including:
• You have a change in residence.
• You have Medi-Cal or Medicaid.
• You are eligible for extra help with Medicare Prescriptions.
• You live in an institution (such as a nursing home).
• You are aging into Medicare (turning 65).
Individuals are eligible to enroll in CareMore Health Plan If:
• You are entitled to Medicare benefits under Part A, and enrolled in Medicare Part B. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party.
• You Reside in CareMore’s Service Area
If you obtain routine care from out-of-plan providers neither Medicare nor CareMore Health Plan will be responsible for the costs. Eligible beneficiaries must use network pharmacies to access their prescription drug benefit except under non-routine circumstances when they cannot reasonably use network pharmacies.
Medicare beneficiaries may also enroll in CareMore Health Plan through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.medicare.gov. For more information contact CareMore Health Plan at 1-877-211-6614.
You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for getting extra help, call:
• 1-800-MEDICARE (1-800-633-4227). TTY/TDD users should call 1-877-486-2048, 24 hours a day/7days a week);
• The Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY/TDD users should call, 1-800-325-0778;
•Your State Medicaid Office
You may enroll for one of CareMore's products by calling CareMore at 1-877-211-6614, downloading an application or completing an application online below. By choosing from the "Location" and "Plan" options below you will begin the actual enrollment process for the Medicare Advantage plan selected. The enrollment process is not complete until your application has been submitted, reviewed and approved.
By submitting this online application form you are sending an actual enrollment request to CareMore Health Plan.
