Job Description:
The Director of Clinic Programs works under the direction of the Medical Officer and is responsible for overseeing Chronic Disease Management programs and the Nurse Practitioners working in CareMore Care Centers.
Essential Duties and Responsibilities Include:
• Oversee and direct the quality of care of patients with chronic conditions; responsible for the quality of care provided by Nurse Practitioners.
• Provide resources and direction to the Clinical Managers and NPs in the clinics.
• Utilize clinical expertise to develop programmatic approaches to care provided for each chronic condition.
• Evaluate clinic staffing and scheduling to prevent delays in patients accessing services.
• Monitor all programs to ensure a high standard of program effectiveness and compliance with protocols.
• Research and development of new and innovative approaches for care and treatment of chronic conditions, in collaboration with clinical leadership.
• Support and implement company initiatives to support growth and other strategic objectives.
• Development of a patient focused and team oriented culture.
• Mentors and develops staff and leadership.
• Provide a working environment with good employee morale and retention that also attracts new candidates based on its reputation.
• Collaborate closely with other Directors of Clinic Programs to evolve the entire Nurse Practitioner organization.
• Foster interdisciplinary approach to care with other medical and clinical disciplines to ensure optimal clinical outcomes and patient quality of life.
• Communicate routinely with other Clinical Directors and Medical Officers and Directors.
• Identify and collaborate to resolve operational issues in the organization.
• Manage the annual budget.
• Evolve the role and infrastructure for Nurse Practitioner programs to coincide with needs and CareMore’s growth.
Education, Experience and Qualifications:
1. Completion of Family Nurse Practitioner Program from an accredited school by the State of California, Board of Consumer Affairs, National certification, Masters Degree in Nursing, Registered Nursing License, Furnishing License and BLS
2. Knowledge of CareMore Care Center protocols and policies
3. Advanced assessment skills, experience in an intensive care setting or related field.
4. 3-5 years management experience.
5. Familiar with standard concepts, practices, and procedures within a particular field.
6. Experience working on Electronic Medical Records System a plus.
7. Strong communication skills/fluency in English language.
8. Experience and knowledge of HCC coding and RAF score, and HEDIS and Star Measure a plus.
