Chronic Obstructive Pulmonary Disease

The Pulmonary Care program provides support for those living with asthma, chronic bronchitis, emphysema and chronic obstructive pulmonary disease (COPD). The program provides you with enpowerment skills and self-management techniques that can be applied immediately to your daily routine. Additionally, you will receive a personalized care plan created by CareMore's expert interdisciplinary care team comprised of a Nurse Practitioner, Primary Care Physician, Pulmonologist,  Social Worker and Case Manager. Your Pulmonologist will manage the coordination of your care in relation to your personalized plan of pulmonary care. The remainder of your care team will provide additional services that support and maintain your overall health and wellness and improve your quality of life. CareMore will help you understand your pulmonary condition and its implications and how to live well and “breath easy”. Program services include:

• Healthy Start: Comprehensive pulmonary medical assessment

• Specialized breathing test to determine the age of your lungs

• Self-management skills training on how to deal with complications

• Medication management
