As a pioneer in whole-person health and value-based care for complex and chronic disease, CareMore has a strong interest in promoting healthy food and nutrition for its members. Our programs include member education and individual counseling by dietitians, available in the clinic, at home and virtually.
If the patient is not eating a healthy diet or has no access to nutritious food, CareMore offers dietary counseling as part of the overall plan of care. Counseling includes education and practical tips on making healthy food choices and cooking nutritious meals. Dietary plans are customized to the patient’s needs. We follow up to answer questions and ensure the patient is staying on a path of better food choices and healthy eating.
Evidence suggests that Food as Medicine programs work. Positive outcomes for patients include better overall health, sense of self-worth and self-efficacy, positive emotional changes, better management of chronic disease and lower incidence of hospitalization.
