Medicare Advantage, otherwise known as Medicare Part C is an alternative option to traditional Medicare. Medicare Advantage plans include the features of Medicare Part A (Hospital), Part B (Medical) and Part D (Prescriptions).
Medicare Advantage plans are run by private companies. They must provide all the benefits and follow the rules required by Medicare. As an all-in-one solution, Medicare Advantage plans often also offer additional benefits that traditional Medicare members don’t have access too. These plans can also be customized to meet the needs of different populations.
Special Needs Plans are Medicare Advantage plans that keep premiums low and don’t require extra costs for Medicare Part D or Medicare Supplement premiums. There are several different Special Needs Plans including I-SNPs, D-SNPs and C-SNPs.
I-SNP: Institutional Special Needs Plan
An I-SNP (Institutional Special Needs Plan) provides customized benefits and care delivery to best meet the needs of people living in long-term care settings, like assisted living communities, long-term care and skill nursing facilities, and group homes and includes a model of care to provide preventive care that supports healthy aging.
D-SNP: Dual-eligible Special Needs Plan
D-SNPs (Dual Eligible Special Needs Plans) are a type of Medicare Advantage plans for people who meet the requirements of both Medicare and Medicaid. D-SNP members often face unique health and home-life issues and these special needs plans can be convenient, effective healthcare options.
C-SNP: Chronic Conditions Special Needs Plan
Medicare has several plans that are designed for certain populations with special needs. One of those plan types is the C-SNP: The Chronic Condition Special Needs Plans. C-SNPs are for those who have one or more severe or disabling chronic conditions and tailor their benefits, provider networks and drug formularies to best meet the specific needs of the individual member
