PHOENIX, Ariz. – January 28, 2019 -- CareMore Health and Cigna-HealthSpring have collaborated to provide comprehensive primary care and chronic disease management services to Cigna-HealthSpring Medicare Advantage (MA) customers in select plans in Arizona. With the collaboration, Cigna-HealthSpring customers in Maricopa and Pima counties who reside in institutional and assisted living facilities will now have access to CareMore’s high-touch model and dedicated team of clinicians who provide care directly to patients in the facilities.

Cigna-HealthSpring’s Arizona Medicare customers who reside in institutional or assisted living facilities who select CareMore Health as their provider will receive comprehensive health assessments, chronic disease management, support with medication management, wound care and other needed medical support from CareMore doctors and nurse practitioners who make regular visits to the facilities. The goal of the program is to keep these customers out of the hospital through personalized and consistent care.
