How does the CareMore Health care model work?

Each patient’s primary care physician helps lead a team that works together to coordinate care around the patient’s individual needs.

Providers begin by listening to what matters most to the patient. Through an initial welcome care visit and comprehensive health assessment, the care team learns about the patient’s health history, medications, goals, preventive care needs, and any additional support they may need.

That information helps shape a personalized care plan that may include screenings, medication reviews, referrals, regular check-ins, and other appropriate support.

CareMore Health care model centers patient health goals within a personalized plan connected across care settings.
